Lab diagnosis of Kala-azar
Direct evidence of diagnosis of kala-azar is:
• Identification of amastigotes in giemsa stained tissue/fluid.
• Blood culture for identification of promastigote
• Biopsy of lymph node, bone marrow, splenic puncture.
Indirect evidence of diagnosis of kala- azar is-
• Blood count to indicate progressive leukopenia.
• Serum tests with :
1. ELISA
2. Indirect fluorescent antibodies
3. Complement fixation- positive within 3 weeks
4. Napier test- positive after 3 months
5. Chopra’s test- less reliable
Medium used for culture is blood agar Nicolle-Novy-MacNeal (NNN) media.
